The Hebrew phrase איך חטאתי would be the more appropriate construction for the translation, "How did I miss/sin?", combining the interrogative איך ("how") with the first-person singular perfect form חטאתי ("I sinned").

This construction emphasizes the manner or way in which the sin/miss was done, distinguishing it from "מה חטאתי?", which asks "What did I sin/miss?"
